Developmental Evaluation of the Community Geriatrics Nursing Specialist Model 2021/2022

The Report summarizes the findings of the Developmental Evaluation of the Community Geriatric Nursing Specialist project. One of the goals of the project was to enhance rapid front-line nursing response to older adults living in the community who are at risk, or who have experienced a sudden or unexpected change in their health condition.
