About Seniors Care Network

We are a regional network of dedicated health professionals, sharing expertise and providing specialized geriatric services. Our clinical services include interprofessional comprehensive geriatric assessment (CGA) and interventions, for older adults living with complex health conditions and their care partners, in the Central East Region of Ontario Health East.

​We work in a diverse geographic region, with urban, rural, and remote communities. We are working together to provide person-centred care focused on the goals and needs of our clients and their care partners. Over the past decade, Seniors Care Network has played an instrumental role in the expansion of SGS in the Central East Region and continues to support Ontario Health Teams.

To lead the network of specialized geriatric services and advance integrated, person-centred care for older adults living with complex physical, cognitive, social and mental health conditions (including frailty and dementia) and their care partners in Central East Ontario.

Our Programs

Seniors Care Network is responsible for the organization, coordination and governance of specialized geriatric services (SGS) in the Central East region of Ontario Health East.

What are Specialized Geriatric Services?

Specialized Geriatric Services are defined as a comprehensive, coordinated system of health services that assess, diagnose, care and support older adults living with complex health conditions (including frailty, dementia and multidimensional impairment), and their care partners. These services are provided by interprofessional teams with expertise in care of older adults across the continuum of care including in primary care, hospitals, home care, community service agencies and the long-term care. In addition to interprofessional geriatric teams, SGS are inclusive of the medical sub-specialties of Geriatric Medicine, Geriatric Psychiatry, and Care of the Elderly.

Did you know?​​​

  • 15% of the Region's 1.6 million residents are older adults who may require additional support to remain living at home, and this is expected to grow by 27% over the next ten years.

  • There are almost 96 500 older adults in the Region living with frailty.

  • The majority of older adults live in the community, and only 9% of older adults living with frailty reside in Long Term Care or Hospital settings.

  • Different approaches to care are required for older adults living in rural and urban areas.

In response to the reports of racism experienced by our specialized geriatric service colleagues, Seniors Care Network convened an Equity, Diversity and Inclusion Task Group in June 2020. The Task Group developed an action-oriented framework for addressing racism in specialized geriatric services.

Our Collaborations and Partnerships

Collaboration with colleagues, partners, and stakeholders is essential in to improving the health and quality of life of older adults. Seniors Care Network actively leads and participates in multiple collaborative projects with partners and stakeholders.
