Caregiver Needs Assessment and Support in Primary Care Guidance Document

Developed in collaboration with the Ontario Caregiver Organization, the Guidance Document outlines a 4-step holistic approach towards identifying and supporting the needs of the caregivers of older adults. Developed specifically for Primary Care settings, the resource also proposes a decision-tree to help guide connections with relevant supports and interventions.


The below template has been developed to help providers tailor the recommended practices in accordance with their preferences and service & program availability in their local area/region.
