Developed in collaboration with the Kawartha Lakes Haliburton Ontario Health Team and presented at the Association of Family Health Teams of Ontario (AFHTO) 2025 Conference, our poster features the Haliburton Highlands Family Health Team (HH FHT) as an early adopter of the Seniors Care Network Primary Care Frailty Pathway. The poster highlights key implementation facilitators and the integral role of digital tools in supporting uptake.
