Our Collaborations

Provincial Geriatrics Leadership Ontario (PGLO) Seniors Care Network supports/co-leads various Provincial projects through committee and working group memberships. Learn more about PGLO.

Specialized Geriatric Services (SGS) East comprises of The Regional Geriatric Program of Eastern Ontario (Champlain), the Centre for Studies in Aging & Health at Providence Care (South East), and Seniors Care Network (Central East). These three entities have leadership responsibilities for coordinating specialized geriatric services in their respective sub-regions. Since 2019, the three SGS entities and PGLO have collaborated to create the infrastructure necessary to support the care of older adults living with complex health concerns across the Ontario Health East Region. This infrastructure is also critical to support the Ontario Health Teams in the Region. Learn more about SGS East.

 

Ontario Health East- Adult Day Programs (ADP) Collaborative: Since June 2024, leadership from the Champlain ADP Providers Service Network, Seniors Care Network in Central East, and Victoria Order of Nurses in South East have been collaborating to support and enhance ADP efforts across the Ontario Health East Region.

 

Central East Adult Day Programs (CE ADP) Committee: Seniors Care Network co-chairs the CE ADP Committee comprising of 17 providers across 44 sites. Examples of our collaborative work include the CE Intra-COVID Hybrid Guidelines and the Central East Region ADP Guidelines.

Ongoing Collaborative Projects

Central East SGS Cognition Referral Pathway: Collaborating with Central East Alzheimer Societies and GAIN Regional Leadership to streamline transitions across SGS and improve patient navigation.

Mission Unit- GAIN-SCN Collaborative: A pilot project to provide Comprehensive Geriatric Assessments to select vulnerably housed older adults with memory concerns; Mission United is a social service and primary health care HUB for those experiencing homelessness.

Peterborough Older Adult Advocate: collaborating with GAIN Regional, ASPKLNH, OPP and other local partners to implement and Older Adult Advocate resource in Peterborough to support older adults suspected of physical and/or financial abuse.  

​

Frailty Pathways: Multiple collaborations underway to develop additional sector-specific frailty pathways e.g., acute care, ED, Community Paramedicine. Visit the Frailty page on our website to view existing pathways and resources.
