Stacey Moore

GAIN Regional Clinical Lead

NP Moore is a highly skilled and experienced Nurse Practitioner with over 20 years in healthcare, including six years in critical care. She holds a Master of Nursing degree and an Acute Care Nurse Practitioner Certificate from the University of Toronto and has been a Registered Nurse in the Extended Class since 2009.


Currently serving as the Regional NP Clinical Lead for the Geriatric Assessment and Intervention Network (GAIN) and Lead Nurse Practitioner at the GAIN Clinic at Peterborough Regional Health Centre, Stacey is dedicated to improving care for older adults experiencing frailty, cognitive decline, and complex health needs. She is also a MAiD assessor and provider, supporting end-of-life care with compassion and professionalism.


Beyond clinical practice, NP Moore contributes to process and quality improvement projects including research and education. She co-investigated a nursing-based randomized controlled trial on hypertension management post-stroke and serves has served as guest lecturer at Trent University, sharing her expertise on collaborative healthcare practices. She is a consultant, having presented for the Nurse Practitioner Association of Ontario (NPAO), Biogen, and Roche, focusing on dementia, frailty, and healthcare system readiness. NP Moore is active chair for several GAIN committees as well as provincial work groups created drive partnerships with other Specialized Geriatric Programs across Ontario.

Recognized for her leadership and dedication, NP Moore has received multiple awards, including the “Working in Partnership” award (2022) and nominations for “Rising Star” (2022, 2023), “GEM” (2020), and “Patient-Centered Care” (2020) at PRHC Professional Staff Awards.


Her passion for advancing geriatric care, mentoring healthcare professionals, and influencing health practice and processes makes her a respected leader in her field.
