About Seniors Care Network
We are a regional network of dedicated health professionals, sharing expertise and providing specialized geriatric services. Our clinical services include interprofessional comprehensive geriatric assessment (CGA) and interventions, for older adults living with complex health conditions and their care partners, in the Central East Region of Ontario Health East.

​We work in a diverse geographic region, with urban, rural, and remote communities. We are working together to provide person-centred care focused on the goals and needs of our clients and their care partners. Over the past decade, Seniors Care Network has played an instrumental role in the expansion of SGS in the Central East Region and continues to support Ontario Health Teams.

Our Purpose
To lead the network of specialized geriatric services and advance integrated, person-centred care for older adults living with complex physical, cognitive, social and mental health conditions (including frailty and dementia) and their care partners in Central East Ontario.
Our Programs
The Specialized Geriatric Services (SGS) that fall under the umbrella of Seniors Care Network are focused on older adults whose complex health concerns (including frailty and dementia) threaten their independence and function. The (following - to delete) programs reflect the use of enhanced geriatric expertise, interprofessional teams, and organizational collaboration.

Together, we are sharing this expertise and ways to deliver care that is responsive to the needs of our clients/patients and care partner, which ultimately provides the best health experience for older adults and their care partners.
What are Specialized Geriatric Services?
Specialized Geriatric Services are defined as a comprehensive, coordinated system of health services that assess, diagnose, care and support older adults living with complex health conditions (including frailty, dementia and multidimensional impairment), and their care partners. These services are provided by interprofessional teams with expertise in care of older adults across the continuum of care including in primary care, hospitals, home care, community service agencies and the long-term care. In addition to interprofessional geriatric teams, SGS are inclusive of the medical sub-specialties of Geriatric Medicine, Geriatric Psychiatry, and Care of the Elderly.

15% of the Region's 1.6 million residents are seniors who may require additional support to remain living at home, and this is expected to grow by 27% over the next ten years.

There are almost 96,500 seniors in the Region who may be considered frail.

The majority of seniors live in the community, and only 9% of frail seniors reside in Long Term Care or Hospital settings.

Different approaches to care are required for older adults living in rural and urban areas.

In response to the reports of racism experienced by our specialized geriatric services (SGS) colleagues, Seniors Care Network convened an Equity, Diversity and Inclusion Task Group in June 2020. The Task Group met on a periodic basis to attain a better understanding of this concerning issue, and developed a framework and an accompanying resource guide to help facilitate our ongoing approach. Seniors Care Network is thankful to our colleagues who contributed to formulating this action-oriented Framework, and who shared their lived experiences with racial discrimination during group discussions.
Our work does not end here. We recognize that discrimination comes in many forms and we are committed to working with our programs and partners to foster an inclusive and non-discriminatory culture across Central East SGS.
